PS4 Issue with X button and some HDMI issues

I am having two issues with the game on the PS4

1) When I launch it seems to turn off the HDMI port on my PS4. Sometimes after a long lag this will then restart back to the age rating screen but other times it will just hang like the HDMI has been disconnected. This does not happen on any other games I have.

2) When trying to play through the console the game does not recognize the X button so I cannot start it when I get past the issues in #1. I can play the game fine through remote play on the vita. It is just the DS4 whose x button is not accepted.

Any ideas? I have tried uninstalling and reinstalling with out any luck.

1) Is there something wrong with the HDMI cable itself? If you have another, try swapping it out and see if that does anything.
2) How are you getting past the first issue in this case? Does it not happen every time or did you find a solution that works occasionally?
